Hotfix for version 2.2 includes

Fixes to some more broken camera angles
Additional attempts at fixing broken cutscenes
Added additional cheat for 99 lives (konami code)
Added prompt for successful cheat activations on main menu
Changed 'student' outfit unlock requirement to unlock 'injured' costume instead
Made 'student' outfit unlocked by default
