Added Level of Detail and other optimisations
Fixed player being frozen after restarting via pause menu

I've tried to see why some people spawn inside the hospital and see awkwardly placed doors. The thing is I'm not encountering any of these bugs, so I'm not sure how I'm going to fix it.
If you do find yourself spawning in the hospital (you're meant to spawn outside of it) press the esc key and then restart.
