Update: Valve has since confirmed to The Verge that the listed information is accurate (but not yet comprehensive). They also confirmed both the announcement and start of pre-orders is set for May 1st with a tentative shipping date in June.
Back in December 2018 a trademark filling for "Valve Index" logo showed up, but back then we didn't know what it was connected to.
Valve teased Valve Index just last week, but today they 'accidentally' released some store pages with quite a bit of information. The new leaked render closely matches the leaked prototype image from July 2018.
Then in a recent video by the developers working on Moondust (the Knuckles/Valve Index Controller tech demo) the developers showed off the SteamVR window with the Valve Index icon and the SteamVR settings window with a resolution mention. See UploadVR's analysis of said video excerpts here.

Looking at the created packages for hardware purchases, we believe the following purchase options will be available:
- Each item separately
- Valve Index Headset + Valve Index Controllers bundle
- Valve Index Headset + Valve Index Controllers + Valve Index Base Station bundle
There are currently no packages with non-hardware apps included. We think this app might be the driver for the headset (as it is listed with other drivers as SteamVR DLC). It is also currently in the master list which means all Valve employees automatically get access to it.

The page is largely a placeholder and included Lorem ipsum placeholder text, however it still gave us some information. More importantly, the page displayed June 15, 2019 as the ship date.
